On July 9, 2013, at approximately 7:30 a.m., the Yuma County Sheriff’s Office received a phone call from an Adult Probation Officer in reference to a male subject that was on probation who was threatening to harm himself with a weapon. YCSO dispatched units to the home of Mr. Rafael Adame and also immediately contacted the Crisis Management Negotiator and provided him with the information needed to make contact with Mr. Adame. Immediate attempts by the Crisis Management Negotiator to contact the subject via phone while Deputies were en route to his home were unsuccessful. The negotiator made continuous attempts to make contact with Mr. Adame, who repeatedly answered, refused to speak with the negotiator, and disconnected the calls. After continued attempts to establish communications, Mr. Adame eventually spoke briefly with the negotiator and denied that he had a weapon. During the series of attempted phone calls, Mr. Adame eventually turned off his cell phone and ended communication with the negotiator.
At approximately 7:40 a.m. Deputies arrived at Mr. Adame’s residence and attempted to call him out from his residence. They received no response. At approximately 8:45 a.m., Deputies made entry into the residence. Mr. Adame was not located in his residence but Deputies did locate an empty hand gun case in his room. A search of the immediate area was conducted by Deputies. Once Deputies had cleared the residence and were unable to locate Mr. Adame, the negotiator once again attempted to make contact with him.
At approximately 9:00 a.m., contact was again made by the negotiator with Mr. Adame via cell phone; however he refused to give his location. Using the cell phone’s tracking system; a general location of his whereabouts was obtained. Deputies responded to the 8100 block of south 7E where Mr. Adame was located in a dirt field west of 7E.
Deputies spotted Mr. Adame and approached him on foot. While approaching Mr. Adame, Deputies witnessed him pull a hand gun from his waist or pocket area and point the weapon at himself. Mr. Adame was given multiple commands to drop the gun and failed to comply. Mr. Adame then transitioned and pointed the gun at one of the Deputies and stated, “You drop yours”. At that point, the Deputies feared for their safety and discharged their weapons. Mr. Adame was struck multiple times and fell to the ground. The Deputies approached Mr. Adame and were able to take the gun from his hand.
Deputies requested Rural Metro Ambulance and immediately initiated first aid on Mr. Adame using their issued trauma kits. When Rural Metro Ambulance arrived, Mr. Adame was transported to YRMC where he succumbed to his wounds at 10:04 a.m.
In the course of the ongoing investigation, it has been learned that the hand gun that Mr. Adame was in possession of was stolen within a few days prior to the incident.
A forensic autopsy was performed by Pima County Medical Examiner’s Office on Wednesday, July 10, 2013. Results are pending.
The case remains under investigation and the Deputies remain on paid Administrative leave until the conclusion of the investigation.

At approximately 7:40 a.m., Deputies arrived at the subject’s residence and attempted to call out the subject from his residence; they received no response. At approximately 8:45 a.m., Deputies made entry to the residence. The subject was not located in his residence but Deputies did locate an empty handgun case in his room. A search of immediate area was conducted by Deputies. At approximately 9:00 a.m., contact was made with the subject via cell phone. Using the cell phone’s tracking system, a general location of his whereabouts was obtained. Deputies responded to the 8100 block of south 7E where he was located in a dirt field west of 7E.
Deputies made contact with the subject who was armed with a handgun. The subject was given multiple commands to drop the weapon and failed to comply. During the confrontation, four Deputies discharged their firearms with multiple rounds striking the suspect. Rural Metro Ambulance was called to the scene. The subject was transported to Yuma Regional Medical Center where he was pronounced dead at approximatley 10:04 a.m.
The Yuma County Sheriff’s Office Criminal Investigations Bureau is currently on scene investigating the incident.
